That Which We Call a Rose


What’s in a name? Sometimes nothing much. Sometimes a shift in paradigm.

The Medical Record in its current format was created over a century ago by Dr. Henry Stanley Plummer at the Mayo Clinic. When in the course of human events the Medical Record began migrating from paper folders to computer files, the Institute Of Medicine naturally named the new invention Computer-based Patient Record System (CPRS). The Medical Records Institute chose the term Electronic Patient Record (EPR). Somewhere along the line the “patient” got dropped from the concept and the software used to compose and store medical records became known as Electronic Medical Record and the name EMR stuck.

As the EMR software evolved and started exhibiting rudimentary information exchange abilities and some semblance of “intelligence”, it was felt that a name change was in order. To differentiate the newer and smarter software from the original EMR, the term Electronic Health Record (EHR) was introduced and is now enthusiastically supported by the Federal Government. The term EHR is used in acts of Congress, rule makings from CMS and ONC and Presidential speeches. Since EMR has been around for quite some time, most industry veterans, as well as most doctors, are a bit confused about the new terminology. Is it EMR or is it EHR? Is it just semantics? Would an EMR by any other name smell as sweet (bitter)?

In a recent ONC blog, Peter Garrett and Joshua Seidman argue that there is a significant difference between EMR and EHR. The former is just “a digital version of the paper charts” and “not much better than a paper record”, while the latter is “designed to be accessed by all people involved in the patients care”, including patients, and generally “represents the ability to easily share medical information among stakeholders and to have a patient’s information follow him or her through the various modalities of care engaged by that individual”. This dramatic difference stems from replacing the word “Medical”, which implies disease, with the word “Health” which is “the general condition of the body”. Note that the word “Patient” is still absent. However, Health is supposedly from cradle to grave, while Medical is episodic in nature. Since, no matter what you call it, clinicians are the primary users of this software, would we say that doctors provide Medical Care or Health Care?

When we say that cost of Health Care is sky-rocketing, we don’t usually include costs for clean air, clean water, car seatbelts and gym memberships, and although we all know that an apple a day keeps the viagra cialis online pharmacy pharmacy away, the cost of apples is not included in our Health Care expenditures. To be sure, Medicine, “the science and art dealing with the maintenance of health and the prevention, alleviation, or cure of disease”, does include costs for direct prevention of specific diseases (immunizations) and efforts for early diagnosis of others (screenings). Historically, doctors, nurses and their less formally educated predecessors have been tending to the very sick. It is with this goal in mind that Dr. Plummer’s collaborative Medical record was created, and it is for this purpose that the American Academy of Pediatrics advocated for a Medical home for sick children, and it is Medical care for the sick which the EMR attempted to facilitate, one patient at a time. The EHR represents quite a different philosophy and places new and expanded responsibilities on the Medical profession.

The EHR is intended to serve the healthy as well as the sick, and the President’s vision is that every American should have one, whether that particular American is healthy or not. The ONC vision, shared by many innovators in the field, is that “EHRs focus on the total health of the patient—going beyond standard clinical data collected in the provider’s office and inclusive of a broader view on a patient’s care”. That broader view will presumably include lifestyle habits, diet and exercise and everything one may wish to record which pertains to one’s general health status. If and when a person becomes ill and is transformed into a patient, the various clinicians and care providers will contribute their documentation to the EHR, and since EHRs are easily shareable, all Medical care will be coordinated through the EHR and collaboration will flourish, as it should. This sounds almost exactly the same as what a Personal Health Record (PHR) is supposed to be. Is an EHR really a PHR?

No. EHRs include one feature that is not possible in a PHR: the ability to aggregate individual patients into populations. When physicians write introspectively about their work, you usually find stories about this or that particular patient, pondering whether they did too much for the 90-year-old Alzheimer’s victim, or too little for the misdiagnosed 40-year-old ovarian cancer patient. They talk about emotions, or lack thereof, about small victories and exasperatingly “non-compliant” middle aged executives who should know better. Each story has a patient with a name, physical details and most often character description. For those inclined to self-assessment, the day-in and day-out tally of these personal episodes is the decisive yardstick. When people recommend a doctor to a friend, they usually talk about “nobody could figure out what was wrong with Katie; he took one look at her and knew right away” or “you can get in today or tomorrow and she is so nice and patient ; always takes my phone calls and you know I can be a pain (giggle)” or “when Adam had that knee problem, he fixed it like magic and he did surgery on cousin Joe’s shoulder; as good as new, and I think he takes care of the Cardinals too; he is definitely the best in town”.

This unscientific, anecdotal method of both performing and assessing one’s work will be replaced by the broader view of EHR enabled population indicators and considerations. Instead of dealing with Mr. Wilson’s gout and Mrs. Wilson’s incontinence, you are now the keeper of the Health of Populations. The EHR can tell you that half of your under 40 patients are obese and doing absolutely nothing about it. You, or your team, will need to intervene because an ounce of prevention today will lead to healthier lives for this population, and lower costs for society. While managing Mrs. Wilson’s neuropathy is important (especially to Mrs. Wilson), having your population of 300 diabetics controlled within acceptable cost effective parameters will become the main focus of your practice. EHRs will provide you with the intelligence (information) to manage your numbers and with ongoing measurements to assess your performance against goals, and EHRs will continuously collect data for ground-breaking research and more effective recommendations.

EHRs, as imperfect, ineffective and downright primitive, as they are today may be our first glimpse of a future where curing or treating disease is largely a thing of the past. If populations are proactively managed and everybody gets their shots and recommended genetic therapy, or whatever they will come up with next, Medical care will be limited to trauma and exotic ailments that have not been researched just yet. While our generation will not be crossing the River into the Promised Land of perpetual Health, it is up to us to manage this transition so human dignity is preserved and collateral damage is minimized in the process of industrializing medicine, a process which starts with changing the M in EMR to the H in EHR.

Totalitarianism is a term employed by political scientists, especially those in the field of comparative politics, to describe modern regimes in which the state regulates nearly every aspect of public and private behavior.
The most influential scholars of totalitarianism, such as Karl Popper, Hannah Arendt, Carl Friedrich, Zbigniew Brzezinski, and Juan Linz have each described totalitarianism in a slightly different way. Common to all definitions is the attempt to mobilize entire populations in support of the official state ideology, and the intolerance of activities which are not directed towards the goals of the state, entailing repression or state control of business, labour unions, churches or political parties. Totalitarian regimes or movements maintain themselves in political power by means of secret police, propaganda disseminated through the state-controlled mass media, personality cult, regulation and restriction of free discussion and criticism, single-party state, the use of mass surveillance, and widespread use of terror tactics.
Critics of the concept say that the term lacks explanatory power. They argue that governments which may be classified as totalitarian often lack characteristics said to be associated with the term. They may not be as monolithic as they appear from the outside, if they incorporate several groups, such as the army, political leaders, industrialists, which compete for power and influence. In this sense, these regimes may exhibit pluralism through the involvement of several groups in the political process.[1]

Usage of the term
The term, employed in the writings of the philosopher Giovanni Gentile, was popularized in the 20th century by the Italian fascists under Benito Mussolini. The original meaning of the word as described by Mussolini and Gentile (G. Gentile & B. Mussolini in "La dottrina del fascismo" 1932) was a society in which the main ideology of the state had influence, if not power, over most of its citizens. According to them, thanks to modern technologies like radio and the printing press, which the state could, and probably would, use to spread its ideology, most modern nations would naturally become totalitarian in the above stated sense.
While originally referring to an 'all-embracing, total state,' the label has been applied to a wide variety of regimes and orders of rule in a critical sense. Karl Popper, in The Open Society and Its Enemies (1945) and The Poverty of Historicism (1961) developed an influential critique of totalitarianism: in both works, he contrasted the "open society" of liberal democracy with totalitarianism, and argued that the latter is grounded in the belief that history moves toward an immutable future, in accord with knowable laws. During the Cold War period, the term gained renewed currency, especially following the publication of Hannah Arendt's The Origins of Totalitarianism (1957). Arendt argued that Nazi and Stalinist regimes were completely new form government, and not the updated versions of the old tyrannies. According to Arendt, the source of the mass appeal of totalitarian regimes was their ideology, which provided comforting, single answer to the mysteries of the past, present and future. For Marxism all history is the history of class struggle, and for Nazism all history is the history of race struggle. Once that premise was accepted by public all actions of the regime could be justified by appeal to the Law of History or Nature.[2]

The political scientists Carl Friedrich and Zbigniew Brzezinski were primarily responsible for expanding the usage of the term in university social science and professional research, reformulating it as a paradigm for the Soviet Union under Joseph Stalin as well as fascist regimes. For Friedrich and Brzezinski, the defining elements were intended to be taken as a mutually supportive organic entity comprised of the following: an elaborating guiding ideology; a single mass party, typically led by a dictator; a system of terror; a monopoly of the means of communication and physical force; and central direction and control of the economy through state planning. Such regimes had initial origins in the chaos that followed in the wake of the World War I, at which point the sophistication of modern weapons and communications enabled totalitarian movements to consolidate power in Italy, Germany, and Russia.

[edit] Criticism and recent work with the concept
In the social sciences, the approach of Friedrich and Brzezinski came under criticism from scholars who argued that the Soviet system, both as a political and a social entity, was in fact better understood in terms of interest groups, competing elites, or even in class terms (using the concept of the nomenklatura as a vehicle for a new ruling class). These critics pointed to evidence of popular support for the regime and widespread dispersion of power, at least in the implementation of policy, among sectoral and regional authorities. For some followers of this 'pluralist' approach, this was evidence of the ability of the regime to adapt to include new demands. However, proponents of the totalitarian model claimed that the failure of the system to survive showed not only its inability to adapt but the mere formality of supposed popular participation. Its proponents do not agree on when the Soviet Union ceased to be describable as totalitarian.
The notion of "post-totalitarianism" was put forward by political scientist Juan Linz. For certain commentators, such as Linz and Alfred Stepan, the Soviet Union entered a new phase after the abandonment of mass terror on Stalin's death. Discussion of "post-totalitarianism" featured prominently in debates about the reformability and durability of the Soviet system in comparative politics.
As the Soviet system disintegrated in the late 1980s and early 1990s, opponents of the concept claimed that the transformation of the Soviet Union under Mikhail Gorbachev, and its subsequent sudden collapse, demonstrated that the totalitarian model had little explanatory value for researchers. Several decades earlier, for example, Bertram Wolfe in 1957 claimed that the Soviet Union faced no challenge or change possible from society at large. He called it a "solid and durable political system dominating a society that has been totally fragmented or atomized," one which will remain "barring explosion from within or battering down from without." Many classic theories of totalitarianism discounted the possibility of such change[citation needed]; however, later theorists not only acknowledged the possibility but in fact encouraged and welcomed it[citation needed]. Any suggestions of the indefinite stability of states labeled totalitarian among proponents of the term were largely discredited when the Soviet Union fell by the wayside.

While the term fell into disuse during the 1970s among many Soviet specialists, other commentators found the typology not only useful for the purposes of classification but for guiding official policy. In her 1979 essay for Commentary, "Dictatorships and Double Standards", Jeane Kirkpatrick argued that a number of foreign policy implications can be drawn by distinguishing "totalitarian" regimes from autocracies in general. According to Kirkpatrick, typical autocracies are primarily interested in their own survival, and as such have allowed for varying degrees of autonomy regarding elements of civil society, religious institutions, court, and the press. On the other hand, under totalitarianism, no individual or institution is autonomous from the state's all-encompassing ideology. Therefore, U.S. policy should distinguish between the two and even grant support, if temporary, to non-totalitarian autocratic governments in order to combat totalitarian movements and promote U.S. interests. Kirkpatrick's influence, particularly as foreign policy adviser and United Nations ambassador, was essential to the formation of the Reagan administration's foreign policy and her ideas came to be known as the "Kirkpatrick Doctrine."[3]

    Voices of experience are essential when establishing governance processes, when initiating and improving governance within the realm of IT this should include all senior management views, including the experienced IT view. Director level skill, knowledge and experience is important in getting it right. To move forward with governance the organizations underlying assumptions need to be challenged and processes improved to accommodate changes brought by rapidly changing IT. A review of double loop learning and the learning organization can assist.
    • Collaborate with the appropriate departments to assess and recommend technologies that support company organizational needs.
    Assessing and recommending technologies requires having an overall knowledge of the organizations IT infrastructure, its active projects and running software. This is also best done with an IT portfolio management approach combined with capabilities-driven IT strategy, for these include great insight into all current IT and near to release software and work toward increasing overall IT (and business) success.
    • Establish a governance process that meets government, partner, and company expectations for customer information privacy.
    Keeping an eye on this is very important. Most jurisdictions have information and personal privacy laws and these need to be respected.     Someone needs to be responsible for evangelizing the company's technology strategy. This should not fall to another C-level role(s) as their priorities and voice is focused on other areas of the business. Director of IT needs to be seen as responsible and owning the technology strategy, communicating it is the best way for this to be done.


    Implementation & Deployment



    • Select and set up web-based internal communications systems, such as a wiki, blog, chatroom, project management, and bug tracking systems (in the absence of a system administrator).
    This should encourage transparency on all internal projects. Through openness dialogue increases and people have a better opportunity of being aware of all activities. These repositories contribute to the business / collective intelligence and knowledge management for the organization, all these sources should be indexed with internal search engine. If system administrator is available they should be made responsible for selection and setup.
    • Collaborate with the senior management, staff and potential customers to develop use cases (or user stories) and specifications that describe the implementation of the company’s services as mobile and internet applications.


    Being Agile in your approach to systems development is very important to your organisations nimbleness and success. Engaging the decision makers and customers when designing and implementing is also insanely important. The Director of IT should champion the gathering of use cases / user stories and use these to drive development.
    • As a member of the senior management team, establish a customer service and support process, with particular responsibility for mobile and web-based services that implement the support function.

    Using wireframes is also a great way to engage in a discussion about what is important within the User eXperience (UX) and how the software is supposed to work. It also allows important information to be discovered before the development effort gets to far down the path. Wireframes save time and money.
    • As a member of the senior management team, participate in the selection of a graphic designer(s) who will create the company corporate identity and design the web application’s look and feel. Ensure that the graphic designer(s) have sufficient web experience, follows guidelines established in the User Experience design process, and delivers assets that can be easily adapted to mobile, tablet and web requirements.
    This is about getting the right mix of design savvy and technical abilities. Having the Director of IT of other senior IT person to assess the designers technical abilities, particularly as they relate to mobile and tablet. Things are changing rapidly and UX is changing rapidly with the emergence of new devices and UI standards. Getting the correct designer is very important. The selection process needs to include investigation of the candidates ability to follow guidelines and be aware of industry standards.
    • Participate in the selection and registration of the company’s web site domain names, including any related-but-unused domains that could compromise the integrity of the business (through competition, typosquatting, etc.).
    Protecting brand and driving traffic to the companies site is very important. This will also become a part of the Search Engine Optimization as driving traffic becomes increasingly important. 
    • Select and set up a software revision control system and repository (in the absence of a system administrator).
    Good version control encourages collaboration. Versioning should be a practice applied to software code as well as database creations scripts and design documents.     • Review current best practices for the selected mobile, tablet, web and application frameworks and establish the architecture(s) for all applications.
    This is about finding the best architecture for each and all of your product offerings. Its about having one person responsible in ensuring all what you offer online works together, is costing the least it can, performs well and exceeds the customer expectation. The one person responsible for all this should be the Director of IT while working closely with solution / infrastructure architect(s).
    • Select and manage company staff or outsourced vendors who will implement the applications.
    Having the right people, in the right place, at the right time takes experience. Identifying these people and managing to their strengths takes good leadership. Having an experienced Director of IT with proven leadership abilities goes a long way in putting together teams to complete application implementation.
    • Develops business case justifications and cost/benefit analysis for technology initiatives. Prepares request for proposals (RFP's) for technology equipment and services.
    Having IT initiatives grounded in business cases and cost / benefit analysis helps to build understanding, create the discussion and the supporting documentation to determine where all technology initiatives fit within the organization IT portfolio. Once the projects are identified that provide the greatest business benefit these can managed within the organizations IT  or be put out to RFP to determine an appropriate vendor. RFPs well managed can provide insight into potential solutions as well as the potential vendors.
    • Liaise with vendors and service providers to ensure efficient and cost-effective acquisition of technology purchases; oversee warranties and service agreements.
    Working with technology vendors and service providers is a partnership that can become more valuable through time. Technology solutions fit better when they are made with knowledge (both historical and current) of the workings and strategy of the business. Having alignment of these with technology purchases, warranties and agreements assist in having good partnerships and integrated solutions.  
    • Establish and supervise the software development process, setting short-term objectives and assessing progress as defined by the selected software development methodology.
    Identifying and establishing the heartbeat for IT is the Directors responsibility. All software development teams establish a heartbeat based on how frequently they deliver software with how many features. Having a solid software development methodology encourages this kind of consistency. Communicating the themes and objectives of each new release should be aligned with the business strategy and IT portfolio.
    • Conduct code reviews and specification conformance testing as defined by the selected software development methodology.
    Code reviews and testing is an essential part for great software. This needs to be championed by a senior IT person and the Director is best as champion. I strongly believe that an Agile software development methodology is the best way to go, therefore the code review would be done via the pair during their pair programming (or whatever the teams derivative of pair programming becomes). Having dedicated testers that are not the programmers is a good idea. The tests should be derived from the user stories and automated. Taking a Test Driven Development approach is a really really good idea.




    • Establish and supervise a quality assurance process, including integration and system testing.
    This responsibility is closely tied to the previous one as code reviews and testing is a part of quality assurance. How this differs is that code reviews and testing at the development (or programmer) level are focused upon each unit, feature and / or use case. The level of quality assurance within this responsibility has to do with the system as a whole from system, integration and user experience testing. It is often a good idea to move these quality assurance activities far away from the application software developers. To be thorough here the Director of IT could move this aspect of quality assurance out to the other areas of the business, where these users work with professional testers to complete the final rounds of testing.
    • Select, deploy, and monitor performance profiling tools and procedures.
    They say, "if you can measure IT, you can manage IT."     • Support the marketing process by providing implementation of technical requirements for Internet marketing and search engine optimization.
    Constantly align and optimize the website with the marketing initiatives, what can be found through one media, should be found from another online source. Driving traffic from multiple  sources offers business benefit.
